Initiate the application

Go to DSO Website (https://www.dsontario.ca/) and click on the “Register” link on the left of the page.

This takes you to the application form (https://www.dsontario.ca/members/register) Fill in the blanks and make sure the DSO region is selected correctly. The form does not always detect the region properly.

The “user account” created here is only used for initiating the application and has no other use to my knowledge. It didn’t give me any updates on the application process.

Submit Documents

Within a few days, an email should be sent to require submitting the necessary documents via email or fax:

  • A copy of OHIP card (front)
  • A copy of latest psychological assessment

Receive Eligibility Letter

Within 30 days, a letter would be mailed to the address provided in the application to inform the eligibility determination.

If the child is eligible for Passport Funding, he/she would get the minimal amount of CDN $5,000.00 annually.

Assess for Passport Funding Amount

A few months before the child turns 18, DSO assessor would contact to make appointments for assessments. It would be broken down to 2 or more sessions for interviews. This assessment is to determine the needs and possibly increase the amount of Passport Funding.

Receive Funding Package

Passport Funding Coordinator would call or email regarding the funding package around the child’s 18th birthday. The reimbursement process is similar to SSAH and MyDirectPlan can be used.
